About This Role
We are seeking an experienced CCP Supervisor to join our team. The ideal candidate will oversee home care service delivery and ensure the highest quality of care for our clients.
Responsibilities
- Manages home care service delivery and case coordination
- Oversees caregiver performance and provides mentorship
- Conducts quarterly home visits to assess client needs
- Maintains communication between clients, families, and caregivers
- Ensures compliance with all regulatory requirements
Qualifications
- High School Diploma minimum (Associate's degree preferred)
- Valid driver's license and reliable transportation
- 2+ years of supervisory experience in healthcare
- Bilingual abilities preferred (English/Spanish)
- Strong organizational and communication skills
